• Your name, address, telephone number, and email address.
• A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
• A statement, under penalty of perjury, that the information is accurate and that you are the owner or authorized to act on the owner’s behalf.

Counter-notice

If you believe material was removed or disabled by mistake or misidentification, you may send a counter-notice to the same email. Your counter-notice should include your signature, identification of the removed material and its prior location (URL), a statement under penalty of perjury that you believe the removal was a mistake, your contact details, and consent to jurisdiction where required.
